All of the below scenes can take place regarding the player character's gender (unless specified otherwise.) Sexual assault, bestiality and child abuse is discussed at several points throughout the game's story in dialogue or in-game books. A myriad of enemies in the game are named or have their genitals exposed. Some characters have erotophobia (fear of sex) and are put at an excessive disadvantage against these enemies. Most of these enemies can have their genitals targeted for attack and "destroyed" (mutilated) by doing so. Some also attack the player with them. If the player interacts with a buzzing object in one of the houses, it summons a "Genie". If the genie defeats the player, a special scene occurs where it beat them to death, then reaches up into their anus to extract their soul (likely a reference to certain spirits from Japanese folklore.) If the player plays as a specific character (Abella), if they attempt to sleep in a bed in the Mayor's Manor, there is a chance they will be woken up by a criminal character attempting to sexually assault her. A fight will break out, throughout which the enemy in question will not be wearing any pants. (No special game over scene occurs if the player is defeated in this fight.) An enemy called "The Woodsman", a naked and disheveled man, has a special attack, where his penis detached in the form of a parasitic insect and latches onto the player's face, sprouting tendrils down their throat. It remains there until death. If the player fails to avoid a special grab attack of the Decrepit Priest enemy, the screen will fade to black. When the player re-awakens, they will be stripped naked, crucified, and their legs cut off. There is a chance for a character to come along and rescue them from this fate, however. A male character will often make sexual remarks towards the female character Abella, regardless on if the player is playing as them. Later, this same character will kidnap another female character and keep them locked up in his Mansion. The player may optionally rescue her (it is unclear what happens if the player does not intervene.) When a particular clown enemy defeats the player, they are taken to a basement where they are shackled. A nondescript red spirit will approach the player and phase into them, at which point the player character will feel compelled to masturbate until passing out. When killed by the enemy "Stitches", there is a special scene which features the player character being sewn into a mass of other human beings, with it being implied that their mouth is sewn directly onto the anus of one of the other individuals (likely a reference to The Human Centipede.) The character "Pocketcat" is implied to be a pedophile. He makes frequent sexual references, is implied to be masturbating constantly and when he (wrongly) presumes one of the characters was being sexually abused by their father, he makes a series of crude jokes at their expense. When the player first meets him, he is seen hunched over a bag. When the child inside the bag begins crying, he beats it into silence. He has also stalked another character, Daan, from early childhood. In the backstory for the playable character Daan, it is stated that his parents were devoted to the Goddess of Love, whose worship often involves ritualistic orgies. While these rituals are usually consensual, it is implied they tried to involve him with their practices when he was a child (by sexually abusing him.) The character Samarie is obsessed with the playable character Marina and the player may be woken up by her watching them if they choose to play as that character. While in an underground area, the player may be ambushed by a gang of Ratkin (mutated rat people.) If the player uses a special skill to understand the speech of these creatures, they are bickering about cannibalizing the player. One of the ratfolk mentions a desire to stick something inside the human's anus.
