A man enters the showers to watch the women. A woman (Michonne) is raped multiple times. The scene is very explicit. Upon seeing a child tied up in her abuser's (The Governor) apartment, the woman believes the child is a sex slave. The woman takes revenge by mutilating her attacker and gouging out one of his eyes with a spoon that she had previously inserted into his anus. Rape is used to make the story dark throughout the series.
